Dear District IV Colleagues On behalf of Lorain Admiral King High School, I invite you to participate in this year’s OMEA Middle School Large Group Adjudicated Event. The event will be held this year on May 14th and 15th. As you prepare for the event, please confer with your colleagues and plan with them the times you will ask for. All requests will be honored to the best of our ability. Please note any special scheduling considerations in your email when you send me your entrance excel file. Concise Most Important Info – (details below) 1. Application materials are to be downloaded from www.OMEA4.com 2. Application is done in two steps: 3. 1st email me a completed excel file downloaded from our web site ASAP 4. 2nd follow up by sending me the hard copy, check and membership proof via snail mail 5. Fee is $120 per ensemble 6. Send me an email at dustinkwiley@yahoo.com so I can add you to my email list. 7. Know the rules and be current with your OMEA membership OMEA membership is required. A copy of your membership card must accompany your application materials. If you are not currently a member, contact our OMEA President-Elect Michael Gallehue at H - (937)339-4610 or mgallehue@woh.rr.com to obtain membership application materials. Please make sure you are familiar with the latest edition of the rules and regulations for this event. Edition 14, revised for 2002, is available from OMEA Vendor John Purdy, PO Box 1039, Troy, OH 45373. Please combine all entries into one check made payable to Treasurer, OMEA District IV and include it with your application. If a copy of a school purchase order is sent, the check must be received before you compete or you will not be permitted to perform. Important!! This year I am again asking that you go to our district web site and download the LGAE Entry Information File and enter your information into it using Excel. Instructions for filling out the file are in the excel file. Email the file to me as soon as you have it prepared (no later than Saturday April 17). DO mail the completed application afterward along with your check for entrance fees and a copy of your membership card. The mailed application form will be used to verify entrance information I receive from email and kept as a record of having the proper signatures. Application forms are no longer mailed to you by OMEA. Application forms are available on our district 4 website (www.OMEA4.com), the OMEA web site (www.omea-ohio.org), and published in the September edition of the Triad. All application materials must be postmarked no later than Saturday April 17th.
